Individualized Health Plan Review and Acknowledgement (Parent Portal)
Follow the instructions below to review your child’s individualized health plan(s) in the Infinite Campus Parent Portal.
Review the Health Plan(s)
1. Navigate to the Parent Portal:
Open your web browser and go to Green Bay Infinite Campus Portal.
2. Click the Documents menu.
3. Select the appropriate School Year.
4. Click the Individualized Health Plan to view.
Complete the Review Survey
1. Click the Message Center menu.
2. Click the Surveys tab.
3. Click on the survey to complete.
4. Select your Language.
5. Answer the question.
6. Click Complete.
