The Green Bay Area Public School District is partnering with Bellin Health to provide onsite opportunities for COVID-19 vaccines. There are two dates listed for each site, the second date is for the second dose. 

All students ages 5 and up and their family members are eligible to receive the vaccine. 

COVID-19 Vaccination Clinic Dates & Locations 

Danz Elementary School (2130 Basten Street) - November 22 and December 13, 3:30 p.m. - 6:00 p.m.

Eisenhower Elementary School (1770 Amy Street) - November 23 and December 14, 3:00 p.m. - 6:00 p.m.

District Office Building (200 S. Broadway Street) - November 24, 9:00 a.m. - 12:00 p.m. and December 15, 3:00 p.m. - 6:00 p.m.

Chappell Elementary School (205 N. Fisk Street) - November 29 and December 20, 3:30 p.m. - 6:00 p.m.

Fort Howard Elementary School (520 Dousman Street) - November 30 and December 21, 2:45 p.m. - 5:45 p.m.

Howe Community Resource Center (526 S. Monroe Street) - December 1 and 22, 2:30 p.m. - 5:00 p.m.

Appointments are preferred; walk-ins will be accepted for any openings in the schedule without scheduled appointments. When scheduling an appointment, please mention that the patient is scheduling for a vaccine at a Green Bay Area Public Schools location (mention location and date).

-Parents need to accompany children 15 and under; parental consent via telephone is accepted for ages 16 and 17.
-Please allow 25 minutes for entire appointment from registration to end of 15 minute wait time after vaccination. 
-Please bring insurance cards, if available, as insurance will be billed for administration of COVID vaccine; the COVID vaccine itself is free.
-If there are any allergies to components of the vaccine, wait times after vaccination will be 30 minutes.
-It is best if the patient is well hydrated and has eaten something in the two hour prior to the appointment. Additional information sheets including what to expect post vaccination, and emergency use authorization information will be available at the time of the appointment.
